California Lifeline Service Transfer
Are you already enrolled in California Lifeline and have service with another provider? If so, please have the following information available prior to starting your application to transfer service:
- Full Name
- Address
- Date of Birth
- Last 4 digits of your Social Security Number
- Your CURRENT service provider
IMPORTANT: Selecting the wrong service provider will cause your transfer to fail and you will not be able to reapply with any company for 24 hours.
How to Confirm Your Current Service Provider
- Dial 611 from your mobile device and listen for the provider’s name.
- Call the California Lifeline Administrator at 877-858-7463 (Mon–Fri, 7 AM–7 PM).
- Log in to your CA Lifeline Customer Portal to view your provider information.
- Remove the SIM card from your device and check for the printed provider name.
