Attention Access Wireless Customers!
Your state requires that all Lifeline customers demonstrate continued eligibility to participate in the Lifeline program once every other year. You must provide proof of eligibility by providing proof of participation in a qualifying public assistance program such as SNAP/Food Stamps, Medicaid or Supplemental Security Income (SSI). You may also provide proof of their income-based eligibility.
In order to satisfy this requirement, you will need to submit Proof of Program Participation or Income Eligibility. (Please send photocopies only. Originals will not be returned to you.)
Acceptable proof of eligibility documentation includes:
- Benefit or Program card (SNAP card, Medicaid card, etc.)
- Valid program award letter or acceptance letter
- Program voucher
Acceptable proof of income eligibility documentation includes:
- Prior year’s tax returns (Federal or State)
- Three months of consecutive pay stubs
- Letter from employer verifying income
In order to ensure that your account is up to date, please provide a photocopy of your program participation and return it to Access Wireless at the following address:
