Attention Missouri Access Wireless Customers!

The Missouri Public Utilities Commission requires that all Missouri residents that receive Lifeline Assistance must demonstrate their continued eligibility to participate in the Lifeline program once every other year by providing proof of participation in a qualifying public assistance program such as SNAP/Food Stamps, Medicaid or Supplemental Security Income (SSI).  Missouri residents may also provide proof of their income-based eligibility.

In order to satisfy this requirement, you will need to submit Proof of Program Participation or Income Eligibility.  (Please send photocopies only.  Originals will not be returned to you.)

Acceptable proof of eligibility documentation includes:

• Benefit or Program card  (SNAP card, Medicaid card, etc.)
• Valid program award letter or acceptance letter
• Program voucher

Acceptable proof of income eligibility documentation includes:

• Prior year’s tax returns (Federal or State)
• Three months of consecutive pay stubs
• Letter from employer verifying income
